Yes each CCU needs its own circuit schedule and test results, the CCU submain details should be recorded on the garage CCU schedule and values at the gargae CCU Zs (Zdb) and PSCC recorded.

It's probably easier to put it on a separate schedule but you don't actually need a separate continuity piece of paper if it's only a small installation, but the test results do need to be shown together with the characteristics of the main and sub board fixed adjacent to the board
What the book says your schedule of test results should be based on the model shown, in other words make your own up as long as it resembles something like so if you want to put it on one page as long as you have satisfied the requirements happy days
From memory the paperwork that the schame providers issue have adequate space for the test information for a small installation
