I do not know any more details about this adaptor at the moment, other than those published on our OFT's website:

Insulation has been wrongly positioned and does not cover the connections on the live plug.

There are several different types of travel adaptor about in this style and I've found a couple where pins from more than one country can be extended simultaneously resulting in a risk of shock by direct contact if plugged into an extension lead or similar.
