Management of staff competence

The majority of major incidents that occur have a human failure or series of failures identified as the significant contributing factor? This always leads to the question, was the individual competent to undertake their job? If an organisation answers yes then the follow up question is how do you know that and how can you prove it?

In order to assist with this problem, some of the regulated industries have introduced requirements for organisations to introduce formal competence management systems for safety critical staff. While this is not a requirement in all industries a formal competence management systems does represent good practice, particularly in the engineering and associated professions.
