Introduction
This talk will address modern safety management within the process industry with emphasis on the findings from the Baker Panel report on the BP Texas incident and the Buncefield report from the HSE. It will provide an overview of major accidents since the explosion at the Flixborough plant in 1974 and describe the evolution of safety approaches within the industry.
The current regulatory framework, safety approaches and tools used within the process industry will then be described. The talk will be illustrated by several high-profile incidents which will highlight the need for a holistic approach to managing safety.
